How to check Eligibility-

• Go to the official government website for PMJAY scheme -
• From the homepage's menu, select the "Am I Eligible" tab.
• Enter mobile phone number and CAPTCHA code
• Now, click Generate OTP.
• Enter your home state as well as your name, ration card number, household number, or mobile phone number.
• If you are eligible, your name will appear in the list
• After determining your eligibility from the online portal's list, you should proceed to the Ayushman Bharat Yojana online registration process.

• Proof of age and identity (Aadhaar Card/PAN Card)
• Contact information (mobile, address, email)
• Certificate of Caste
