You can find the option of "Am I eligible" on the page and click on it to check your eligibility easily, if you are eligible for it you have to fill up the form and then afterwards you can check the status of your Ayushman health card. You can also find all the detailed information regarding the health facility and an "Ayushman Card" on the site itself.

The documents required for the card are an income certificate, SC/ST caste certificate, Aadhaar Card and other documents to support your claim of National citizenship and earnings.
