HMPV Outbreak Worsens Around The World, State Govts Issue Advisory; Find Symptoms, Treatment & More

The recent surge in Human Metapneumovirus (HMPV) cases in China has triggered global attention, prompting the Indian health authorities to issue precautionary advisories. While the Union Health Ministry reassures that the situation is under control, it remains vigilant, monitoring updates from the World Health Organization (WHO) and other international health agencies.

What is HMPV?

First identified in 2001, Human Metapneumovirus (HMPV) belongs to the Pneumoviridae family, which also includes respiratory syncytial virus (RSV). It primarily causes upper and lower respiratory infections, with symptoms similar to the common cold or flu.

The virus affects people across all age groups but poses a heightened risk to young children, older adults, and individuals with weakened immune systems. Its seasonal nature typically sees it circulate during late winter and spring in temperate regions.

Symptoms of HMPV

According to the US Centers for Disease Control and Prevention (CDC), common symptoms of HMPV include coughing, runny or congested nose, fever, sore throat, wheezing or difficulty breathing in severe cases. In some instances, HMPV can escalate into more severe conditions like bronchitis, pneumonia, or worsening asthma.

How HMPV Spreads?

HMPV spreads via respiratory droplets expelled during coughing or sneezing. Other modes of transmission include:
Close contact, such as handshakes or touching infected individuals
Contact with contaminated surfaces, followed by touching the mouth, nose, or eyes

Preparedness & Advisory

In response to the reports of rising HMPV cases in China, the Indian Health Ministry convened a meeting on Saturday to evaluate the situation. While there is no immediate cause for alarm, authorities are taking proactive measures.

The Karnataka & Maharashtra state health departments have issued an advisory urging the public to:
Cover their mouth and nose while coughing or sneezing.
Wash hands frequently with soap for at least 20 seconds.
Avoid public spaces if symptomatic.
Wear masks in crowded areas to minimize exposure.
Health officials have assured that there is no increase in HMPV cases in India compared to December 2023.

International Perspective

Beyond China, countries like Malaysia have also reported a notable uptick in HMPV cases. Videos circulating on social media show congested hospitals and strained healthcare systems in affected areas of China.

Despite concerns, Chinese authorities have downplayed the outbreak, emphasizing that the country remains safe for travel.

Testing & Diagnosis

Testing for HMPV is not routine and is typically conducted only in severe cases or during outbreaks. Diagnostic methods include:
Nucleic acid amplification tests (NAAT) are used to detect the viral genome.
Immunofluorescence or enzyme immunoassays to detect viral antigens in respiratory secretions.

Treatment Options

There is no specific antiviral treatment or vaccine for HMPV. Management primarily focuses on alleviating symptoms:
Staying hydrated and getting adequate rest.
Using over-the-counter medications for fever, congestion, and pain relief.
In severe cases, hospitalisation may be required for oxygen therapy or intravenous fluids.

HMPV Vs COVID-19

While both HMPV and COVID-19 cause respiratory issues such as coughing, fever, and congestion, they differ in several aspects:
Seasonality: HMPV peaks during winter and spring, whereas COVID-19 spreads year-round due to evolving variants.
Transmission Dynamics: HMPV's spread is influenced by seasonal patterns, while COVID-19's transmission has been driven by global travel and variant mutations.

A post-pandemic surge in HMPV cases has been observed, with studies suggesting that reduced exposure to viruses during lockdowns weakened immunity, leading to a rise in respiratory infections once restrictions were lifted.

When to Seek Medical Attention?

As per guidelines from the Cleveland Clinic, individuals should contact a healthcare provider if symptoms:
Worsen after a few days.
Include difficulty breathing or cyanosis (bluish skin).
Occur alongside chronic conditions like asthma or COPD.

Dr Lancelot Pinto, a pulmonologist at PD Hinduja Hospital, told the Times of India that HMPV cases in India are not uncommon but are rarely tested due to the high cost of panels, ranging from Rs 15,000 to Rs 20,000. He emphasized that testing is usually conducted only in severe cases.

The state health department has reviewed respiratory infection data and confirmed no unusual increase in HMPV cases compared to previous years.

Preventive Measures

The CDC recommends the following steps to reduce the risk of HMPV infection:
Wash hands thoroughly with soap and water.
Avoid touching the face with unwashed hands.
Stay home when feeling unwell to prevent spreading the virus.
Regularly clean frequently touched surfaces.
Wear masks in crowded spaces, especially during outbreaks.

While HMPV is contributing to a rise in respiratory infections globally, health experts maintain that it is neither a new virus nor a pandemic threat. Seasonal surges, particularly post-pandemic, are part of a broader re-exposure to common pathogens.
